Five error causes worth separating
- source: misread condition, option, mark, or response type;
- model: selected the wrong theorem, representation, or assumptions;
- calculation: setup correct, execution failed;
- format: MSQ set incomplete or NAT entry/precision wrong;
- time: viable method unfinished.
Do not label every wrong response “concept.” Confidence matters too. A high-confidence wrong answer has more diagnostic value than a low-confidence omission because it exposes a stable misconception.
